After missing a second appointment without notifying us 24 hours in advance we will not schedule a third appointment.

We are network providers for Cigna PPO/DMO, Metlife PPO, Humana PPO, Dentex, Delta Dental and Dorsey Dental, however, if you do not see your plan listed please call our office so that we might check your plan benefits to see what arrangements can be made.
We attempt to let patients know, as near as possible, the cost of their treatment. This is accomplished by estimating the patient's out of pocket costs (Co-Pay) by using the information provided to us by the patient and their insurance provider. We make timely attempts to verify valid coverage, coverage limits and coverage balances remaining for the patient, but the accuracy of the estimate depends on the information we have received before the service is provided.
As a courtesy to our patients, we file insurance claims at the time treatment is completed. Claim filing includes the submittal of all necessary x-rays and a narrative report to the insurance provider. We also provide a complete report of diagnosis and treatment to the patient's referring dentist. Although we file the necessary claims and reports for our patients, the ultimate financial responsibility for the cost of treatment belongs to the patient.
We allow a generous 90 day period to collect the balance due from the insurance provider and the patient. If we do not receive payment during this period, patients are advised in writing that final payment is required within 10 days. If the patient does not contact us to discuss unusual circumstances and possible financial arrangements, a collection agency may be utilized to collect the balance due. Collection fees and NSF fees apply to delinquent accounts where applicable by law.
